Medication Reviews
We review your medication on a regular basis. The date of your next review will be situated at the bottom of the repeat request slip.
A medication review will enable us to make sure that you are taking what you need. It is also an opportunity to review your medical condition(s).
If you have an appointment with a doctor and you know that your medication needs to be reviewed soon. Please discuss this with the doctor involved. This may avoid you having to make another appointment simply for a medication review.
What does a medication review involve?
We need to make sure that the medication you are taking matches the medication in your records both, the dose and frequency. If there have been changes,for whatever reason ,we need to record these to make sure there is a valid reason for the change.
Any side effects, interactions will be explored and if need be, the medication can be stopped, dose altered or the medication changed to another, dependent upon the patients circumstances.
At the review will may need to make sure that any relevant tests have been undertaken to ensure that you are well & the medication remains suitable for you. An example of this is when a patient is taking medication to reduce blood pressure, we will not only check the patients blood pressure but also weight,smoking status, alcohol consumption and advise accordingly. We will also check that you have had blood tests to test your cholesterol,blood glucose(to exclude diabetes) kidneys,liver,bone and blood count.
There will be a need to discuss how the patient feels about their medication and on occasion there may be a need to stop or reduce the medicatiuon.
